What to include
- Your name, role and organisation, if relevant.
- The learner's age, year group or post-16 status, and broad area of need.
- Whether the enquiry is from a family, school, local authority, college or professional.
- What kind of support or transition is being considered, including any accepting provider or next-step setting.
- Any thoughts about gaps in local provision or what would make the biggest difference.
